We support people who are planning for their admission to hospital or who are worried about how they will manage following their discharge from hospital. We aim to get you home safely and to connect you to community services that can support your ongoing independence. Do you need some help while you are in hospital and support

to help get things in place for when you return home? Have a friend, family member or loved one that you care for? we can help you as well.

 

Please note we are not a domiciliary care service. We provide practical support, advice and guidance that supports independent living.  Including; housing issues, claiming welfare benefits, managing debt, accessing the right services, reducing social isolation and more
